Return this signed permission slip with $20 to STUDENT SERVICES by Wednesday, June 3. Make checks payable to: PARK VIEW MIDDLE SCHOOL or pay online using the QR code link If you wait to purchase your ticket at the door, it will cost $30 and any party favors may be limited in choice/availability. CELEBRATION EXPECTATIONS Respect Yourself: -Attend the full day of school, the day of the celebration and have no out-of-school suspensions during the 4th quarter -Wear appropriate, special occasion clothes + wear/bring a pair of comfortable shoes -Arrive on time, Doors open at 5:45 pm and close at 6:15 pm -Stay for the full event, Students must stay until 8:30 p.m. unless the child's parent comes in to check their child out with a staff member Respect Others: -Use positive language, compliment others, and maintain your personal space Respect Your Environment: -Leave decorations untouched, pick up after yourself, plan to enjoy food and beverages at the event in the designated areas
